Abstract
The goal of this evaluation plan was to evaluate the 2013 Polk Community Connect program. Polk Community Connect (PCC) is a free health fair geared toward providing services to low-income and homeless individuals. This evaluation will focus on the effectiveness of the services and their ability to improve the health of the participants involved. Research data about fair participants will be collected using different methods. Data collected provides the evaluators with basic demographic information about participants, if their health needs are being met, and any personal viewpoints about the health fair that may be of value to the evaluators.
